• Enter the captcha and click on the submit button.

• On the search page click on the NPPES option on that page.
• The login page or sign-in page will open.

• Enter the Identity & Access Management System (I&A) User ID and password.
• Then, click on the sign-in option.

1. Visit the portal of – https://nppes.cms.hhs.gov/#/
2. Click on the forgot user ID and password option on the page.
3. Then click on the forgot user ID option.
4. The page will open to set a password.
5. Enter the username and click on the continue option to change the password.

2. Then click on the continue option for terms and conditions.
3. Now, click on the retrieve username option.
4. Enter the email ID and click on the continue option.
